· Conduct individual need assessments to identify the most vulnerable cases;

· Provide information & counselling, making referrals (internal and external)

· Communicate with the beneficiaries directly during the case management cycle to provide timely response;

· Monitor, review and follow up the cases which have been provided assistance;

· Collaborate with other members of the project team to meet the needs of the individual and the population;

· Plan for the weekly case management committee and prepare all the related documents and files;

· Preparing the services mapping and sharing updates with other team members.

· Provide reports on all the activities implemented according to the project indicators

· Conducting appropriate data entry for each case.

· Filling Sened files depending on direct line explanation

· Contribute to the situation reports when needed

· Provide a mission report and/or handover document at the end of contract.

· Ensure the documentation of each activity is in line with donor requirements

Identify and register cases with protection concerns, ensuring thorough documentation.

Case Assessment and Planning, conduct assessments to determine vulnerability and risk levels for Child Protection

Ensure quality interventions through regular monitoring, review and follow-up of cases.

Conduct home visits, and provide information sessions when needed

Assist in developing outreach methodologies to effectively reach and engage with vulnerable populations.

Contribute to the development of referral pathways, service mappings and coordination with other stakeholders for external referrals

Advocate with relevant institutions to ensure beneficiaries are able to access rights & services

Maintain confidentiality of data and demonstrate respect towards beneficiaries, adhering to principles of "do no harm".

Work in close coordination with Centre Supervisor, and attend supervision sessions, follow-up meetings and staff train

Responsible for facilitating PSS sessions and social cohesion activities for individuals with and without disabilities.

Develop daily plans aligned with the PSS curriculum, ensuring the availability of necessary materials.

Create and implement PSS tools and resources to showcase beneficiaries' growth and development.

Conduct engaging sessions tailored to specific age groups, encouraging active participation from all individuals.

Ensure adherence to safe learning practices during all activities and sessions.

Administer pre- and post-assessments to track beneficiaries' progress towards their learning goals, ensuring timely completion of assessments, follow-ups, and progress reports.

Maintain accurate and up-to-date records of interactions with beneficiaries and their families, including progress reports and any pertinent.

Support and Collaboration with direct manager, assist in curriculum development, lesson planning, assessments, and objective setting,

collaborating closely with the MEAL team to monitor activities effectively, with existing PSS, social cohesion, and Caregiver Engagement monitoring tools and use PSS selection criteria effectively.

Uphold principles of PSEA (Prevention of Sexual Exploitation and Abuse), promptly reporting any incidents.
