Monitor the overall health of patients receiving dialysis. Central point of contact between the patient, nephrologist and other healthcare providers involved in the patient's care. Prescribe and manage medications that patients undergoing dialysis may require. This includes medications to manage blood pressure, phosphate binders, and other medications needed to support kidney function and overall health. Schedule routine follow-up appointments to assess the patient's response to treatment, adjust medications as necessary, and address any new or ongoing health concerns.
